This page is an example of a template for displaying a standard declaration of consent by means of a form on DTU SharePoint, a tool that the departments can choose to use.
A department-specific version can be adapted to the department’s local wishes and can be ordered by contacting ait-sharepoint-support@dtu.dk
List of considerations and preparations to be carried out before contacting SharePoint Support.
1.Choose language
It is not possible to choose between an English and a Danish version, so the department must decide which language to use in the form.
2. SharePoint Support needs:
- Name of department – you decide whether you want the long or the short name
- Names of the persons that will administer the department's SharePoint list
- The email address to write to in order to withdraw a consent
- The email address to which a copy of the form of consent is sent
If the department needs functional mailboxes, please contact ITService itservice@dtu.dk
It is important that the department ensures that only administrative employees that need it (e.g. in connection with case handling) have access to the declarations.
3. On DTU Inside the following must be made:
- A page with a link that starts the form. The link is provided by SharePoint Support.
A landing page when the form has been filled in - a thank you page. SharePoint Support needs the link to the thank you page.

4. The text used in the form
If the department wants to change the text used in the form, apart from name of department and emails, please contact video@dtu.dk before contacting SharePoint Support
